PDACL Membership Application - Introduction

Membership to PDACL is a serious commitment.  By applying for membership you automatically infer that you agree with the Aims and Philosophy of PDACL as contained in the Missions Statement and Vision of the association.  The association believes in the rights of personhood for all those we serve.  Please refer to these documents before submitting  your application.

Memberships expire on March 31st of each year.· PDACL would like you to renew your membership before the end of March (at the latest, by April 30th).·PDACL is providing two convenient web-based options and a hard copy option for our members to renew their membership and New Members.

Option One: THE ONLINE ONLY VERSION:· This allows you to COMPLETE the form ONLINE, SUBMIT and it will be automatically sent to the Corresponding Secretary.· Contained within this form, there are two OPTIONS for payment - by cheque/cash or by PAYPAL.· Click here to access the·ONLINE ONLY version.

Option Two: THE ONLINE PRINT VERSION: For those who prefer to use a simple print version, this option has been created.· On this page you will see a form ready to be completed.· Once complete you can then PRINT the form.· The information on this form will not be transferred electronically to PDACL, you will need to mail or drop it off to the PDACL office, along with payment.· To access the print version please use the following link: ONLINE PRINT version·

Option Three: If you prefer to complete your membership using paper copies entirely, the following options are available: PDACL Membership Application Form OR Pick up a copy of the form at the PDACL Main Office.
